I authorise the subscription of 99p per week from my salary to be paid to Staff Services until further notice. It is voluntary, non-refundable and may be varied. Also deductions to continue if moving between trusts (Minimum Membership 1 year). Cancellation only being via the Staff Services Office who will forward request to Finance. (Nb. not directly to Finance). Every effort is made to keep your details up to date, but the onus is on the member to keep us informed of home and work addresses changes otherwise we cannot guarantee correspondence & Membership Plus Book to be delivered to the correct address.
